ABSTRACT

Objective:To explore the effect of peripheral blood T lymphocyte subsets on locally advanced rectal cancer after neoadjuvant chemoradiotherapy.Methods:108 patients were included at the Department of General Surgery of Beijing Chaoyang Hospital from Jun 2016 to Jun 2017. Peripheral blood was collected within one week before neoadjuvant therapy and one week before rectal surgery. Flow cytometry was applied to detect the CD3 + 、CD4 + 、CD8 + 、CD45RA + 、CD45RO + expression. Receiver operating characteristic (ROC) curve analysis was performed to determine the best cut-off value of the ratio of lymphocytes. A logistic regression model was obtained in multivariate analysis. Results:The values of CD3 + , CD4 + and CD8 + T lymphocytes in peripheral blood of the patients decreased compared with that before neoadjuvant treatment (all P<0.05). There was no significant decrease in the proportion of CD4 + , CD8 + , CD45RA + T and CD45RO + lymphocytes in patients′ peripheral blood (all P>0.05). The CD45RO in peripheral blood decreases during neoadjuvant therapy for locally advanced rectal cancer, and it is associated with better tumor regression( P<0.05). The best cut-off value for the ratio changes of CD45RO was 1.07. The ratio changes of CD45RO were the only significant factor for tumor regression in multivariate analysis ( P=0.005, OR=26.867, 95% CI: 1.530-471.635). Conclusion:The percentage of peripheral blood CD45RO may predict the sensitivity of neoadjuvant therapy in rectal cancer patients.

ABSTRACT

Objective To investigate the clinical value of dynamic electrocardiogram (ECG),heart color Doppler ultrasound in the diagnosis of the paroxysmal atrial fibrillation .Methods A total of 130 cases of suspected paroxysmal atrial fibrillation in Zhejiang Rongjun Hospital were included in the study .The patients received routine ECG,dynamic ECG,heart color Doppler ultrasound examination ,the examination results were analyzed .The clinical comprehensive diagnosis was used as the diagnostic reference ,the diagnostic sensitivity,specificity,accuracy of three methods in the diagnosis of paroxysmal atrial fibrillation were compared .And the consistency coefficient of three examination results and clinical diagnosis of paroxysmal atrial fibrillation was compared .Results The sensitivity, specificity and accuracy of the heart color Doppler ultrasound in diagnosis of paroxysmal atrial fibrillation were 94.23%,96.15%,94.61%,respectively,which of the dynamic ECG were 97.12%,100.00%,97.69%,respectively, which of the routine ECG were 83.65%,76.92%,82.31%,respectively.The diagnostic sensitivity ,specificity and accuracy of the heart color Doppler ultrasound were higher than those of the routine ECG (χ2=5.915,4.127,9.646, all P<0.05).The diagnostic sensitivity,specificity and accuracy of the dynamic ECG were also higher than those of the routine ECG(χ2=10.843,6.783,17.094,all P<0.05),the differences of the heart color Doppler ultrasound and dynamic ECG were not statistically significant (χ2=1.045,1.020,1.664,all P>0.05).The consistency coeffi-cient of the heart color Doppler ultrasound ,dynamic ECG with the results of clinical diagnosis were 0.735,0.762, respectively,which was higher than that of the routine ECG .Conclusion Dynamic ECG,heart color Doppler ultra-sound in the diagnosis of paroxysmal atrial fibrillation has higher accuracy ,sensitivity and specificity,which can be used as the main means of diagnosis of paroxysmal atrial fibrillation .

ABSTRACT

Objective To investigate the curative effect and safety of haploidentical allogeneic cytokine-induced killer (CIK)in treatment of advanced hepatocellular carcinoma.Methods The peripheral blood mononuclear cell (PBMC) of the healthy immediate family members of 21 patients with advanced hepatocellular carcinoma (HCC) were collected,induced into haploidentical allogeneic CIK in vitro and transfused to the patients for 4 cycles.The curative effect and safety were assessed.Results The 21 patients were followed up for half a year.The survival rate was 81 % (1 7 /21 ).Among the 21 patients,1 1 cases were with stable disease and 1 0 cases were with progressive disease (including 4 dead cases).Six patients developed fever of different degrees during the treatment and one patient developed rash.The platelet counts of the patients at the fourth cycle after the treatment decreased compared with that before the treatment ,with significance difference (P <0.05).The difference in leukocytes,neutrophils,lymphocytes,hemoglobin,liver function and renal function at the first and fourth cycle after the treatment had no statistical significance (all in P >0.05 ).Conclusions Haploidentical allogeneic CIK in treatment of advanced HCC may effectively improve the quality of life and the adverse reactions are tolerable,which is a relatively safe therapy.

ABSTRACT

Objective To determine whether regulatory T cells(Tr)are increased in patients with tuberculosis and whether they are associated with its immunopathology.Meantime,to investigate the possibility of tuberculosis(TB)as a model for studying Tr functions.Methods The lymphocyte subsets were isolated from peripheral blood mononuclear cells by sorting with flow cytometry.Total cellular RNA was extracted and RT-PCR was performed to detect the Foxp3 mRNA in purified CD3+CIM+T cells,CD3+CD8+T cells and non-CD3+CD4+CD8+T cells.Using FACS analysis.we further investigated the distribution of Foxp3+ population in CD4+ CD25+T cells.Finally,we compared the percentage of CD4+CD25highFoxp3+T cells present in 51 active patients with tuberculosis and 40 uninfected healthy control subjects by FACS.The detection of Tr infiltration of Foxp3+ cells were performed with immunohistochemistry(IHC)method on tuberculosis pathological sections.Results Foxp3 was specific expressed in CD3+CD4+T cells,either in tuberculosis patients or healthy control subjects.Foxp3+ T cells took about 85%fraction of CD4+ CD25highpopulation.We used CD4+CD25high Foxp3+as a detective markers for Tr in the FACS analysis.The results showed that patients with active TB had a 4.4 fold higher percentage within the CD4+T cells in peripheral blood compared to healthy control group(modian,1.01%vs 0.23%,P<0.01).Much higher frequency of Tr were found along with T cells infiltration at the tuberculosis pathological tissues.A few individuals that we can followed indicated the expanded Tr was declined after curative treatment with operation.Conclusion Tr cells are increased in tuberculosis patients and closely correlate with its immunopathology.Tuberculosis should be a valuable model for Tr functional study.
